Clinical Characteristics of Spermatic Cord Lipomas

Abstract
Lipoma of the spermatic cord is often found incidentally during inguinal hernia repair. When correcting inguinal hernia, laparoscopic surgeons are concerned about spermatic cord lipoma, which can cause postoperative inguinal bulging. This clinical review describes the prevalence, proper management, and importance of spermatic cord lipoma. Inguinal hernia repair cases between December 2009 and July 2015 were reviewed via electronic medical record systems. All repairs were undertaken via open technique. Sex, weight, height, BMI, and type and location of hernia were compared to identify clinical characteristics of spermatic cord lipomas. During the study period, 928 cases of inguinal hernia repair were performed. Prevalence of cord lipoma is associated with age, body weight and the defect size of inguinal hernia. Failure to recognize and manage spermatic cord lipoma is associated with incidence of recurrent hernias. Even though herniation of preperitoneal fat via the inguinal canal does not involve a hernia sac, it should be considered a variation of inguinal hernia. A spermatic cord lipoma should be removed and the inguinal canal repaired using inguinal hernia repair technique.
